Position: Full-time/Part-time Home Health Registered Nurse (RN)Illinois

Locations: Gurnee and McHenry areas (and surrounding communities)Wisconsin

Locations: Sheboygan, Manitowoc, Eau Claire, Stevens Point and Wausau areas (and surrounding communities)

Competitive Hourly Wage: Up to $45.00/hr.Sign-on Bonus: $2,500.00

Experience: Current RN licensure, Home Health background preferred

Qualifications

Registered Nurse with current license in the state of Wisconsin/Illinois; One year of

experience as an RN in an acute care setting, preferably two years of

experience. Home Care

experience is desired.

ESSENTIAL FUNCTIONS: (Other

duties as assigned)

Performs assessments and develops individualized patient care plans and manages multi disciplinary care as applicable. Provides skilled nursing care in a patient's home as ordered by the attending physician in conjunction with the plan of care.Supervises LPN's and Home Health Aide's in a patient's home.Complies with agency policies and procedures. Promotes and maintains an agency environment that is in compliance with federal, state, and local regulations. Submits clear and appropriate documentation in a timely manner and documents patient

education. Communicates effectively to physicians, supervisors, and coworkers in a timely manner. Collaborating with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients. Participates in case conferences to promote coordination of care Promotes continuity of care with appropriate admissions, transfers and discharges.Keeping abreast of developments in healthcare, attend in-service training in addition attend agency meetings as required.Reports to appointments as scheduled.Maintain a professional appearance
