A Long Island-based nonprofit healthcare organization is looking to add a licensed Registered Nurse (RN) at one of their residential facilities in Suffolk County.
About the Opportunity:Setting: Residential facility
Schedule: Monday to Friday
Hours: 7:30am to 4pm or 10am to 6:30pm
Patient Population: Adult ID/DD OPWDD Patient Volume: Caseload of 20EMR: Epic
Responsibilities
The Registered Nurse (RN) will:Serve as the medical liaison for IDT team and ensure proper medical follow-upAssess injuries/accidents to ensure individuals receive proper medical careDevelop written plans for each person; modify nursing plan, as necessaryMaintain medical records for each individualAccompany individuals to medical appointments, as assignedEnsure proper medical follow-up through an on-call systemEnsures all medication is administered and stored, as necessaryComplete appropriate incident reporting paperwork, as requiredAct as a liaison with family members regarding medical issuesProvide ongoing training and supervision of AMAP certified staffPerform other
duties, as needed
Qualifications:Graduate of an accredited School of NursingNYS Registered Nurse (RN) licenseSolid assessment, clinical, and documentation
skillsPatient orientedGreat interpersonal
skillsExcellent communication
skills (written and verbal)Strong attention to detailHighly organizedDesired
