A nonprofit organization in New York City is currently seeking a licensed Registered Nurse (RN) for a promising opportunity with their team in Manhattan. In this role, the Registered Nurse (RN) will be responsible for assisting clients in transition from hospitals back to the community.
About the Opportunity:Start Date: ASAPSchedule: Monday to Friday
Hours: 9am to 5pm
Caseload: 5 to 7 per day
Responsibilities: The Registered Nurse (RN) will:Assess patient health problems and needs reflective of holistic needs, develop and implement nursing care plans, and maintain medical recordsConsult and coordinate with healthcare team members to assess, plan, implement, or evaluate care plansAdvise patients on health maintenance and disease prevention including effects of diet and physical activity and provide case managementMonitor, record, and report symptoms or changes in patients' conditions and record medical information and vital signsProvide information to other clinical staff, supportive housing staff, and administrators on nursing standards of practice and proceduresProvide health
education/teaching for patients and their families for specific medical needs, preventative care, and how to access and utilize healthcare services in the communityConduct counseling sessions, with direction as needed by other qualified professionals, according to principles of communication, human behavior and psychiatric nursingParticipate in risk management and quality assurance concerning health related issuesProvide emergency care and crisis intervention, as neededHandle medication reconciliation and observe and records effects and monitor for side effectsPerform other related

Qualifications:1+ year of Nursing
experience working with individuals with Mental Health needsAssociate's and/or Bachelor's Degree in NursingNYS Registered Nurse (RN)Knowledge of professional Nursing concepts, principles and practicesKnowledge of concepts and principles from the Biological Sciences that are supportive of nursing practiceKnowledge of concepts and principles of Health Promotion and Disease PreventionKnowledge of Emergency treatments and interventionsSolid assessment, clinical, and documentation
